At Skippack Pharmacy, our Diabetes Self-Management Education and Support (DSMES) program gives you practical education, personalized coaching, and ongoing pharmacist support to make diabetes care feel less overwhelming. Through medication guidance, frequent check-ins, and real-world follow-up between doctor visits, we help improve the safety, effectiveness, and everyday success of your diabetes treatment plan, so you can feel confident, supported, and steadily moving toward better outcomes.

Questions come up when doses change, blood sugars run high or low, side effects pop up, or insurance suddenly changes what’s covered.
Since many patients visit Skippack Pharmacy up to 35 times each year, our team can help spot concerns early, support you through medication changes, and work alongside your doctor to help you stay on track.
What You’ll Receive

When you join the program, our pharmacy team can help with:
Taking Medication
Feel more confident with your insulin, pills, GLP-1s, injections, dose changes, refill timing, and avoiding medication mix-ups.
Monitoring
If you use a meter or continuous glucose monitor (CGM), such as Freestyle Libre, Dexcom, and Stelo, we’ll help you better understand your blood sugar patterns, trends, and what your numbers may be telling you.
We can also incorporate InBody body composition scans to help track changes in body fat, muscle mass, hydration, and overall progress beyond the scale, especially for patients focused on weight loss, metabolic health, or preserving muscle while improving blood sugars.
Healthy Eating
Simple guidance around meals, snacks, timing, and how food choices may affect blood sugar throughout the day.
Being Active
Practical ways to safely build movement into your routine in a way that works for your lifestyle and goals.
Reducing Risks
We help catch concerns early, including low blood sugars, side effects, injection questions, missed doses, and other safety issues.
Problem Solving
Support for the real-life situations that come up—sick days, travel, changing schedules, insurance issues, or unexpected blood sugar swings.

How the Program Works
Getting started is simple, and our team will help guide the process.
1) Talk With Your Doctor
If you’re interested in extra support with diabetes medications, blood sugar monitoring, healthy eating, or progress tracking, the first step is to ask your doctor or diabetes provider for a referral to Skippack Pharmacy’s diabetes support program.
This program is only available for patients with active Medicare coverage at this time, which allows our clinical services team to provide ongoing support and bill the service appropriately.
You can also share this page with your provider so they can learn more about the program.
2) We Receive the Referral
Once your provider sends the referral, our clinical team will review your insurance coverage, medications, recent concerns, and the goals your provider would like us to support.
3) Your Personalized Diabetes Support Plan
Our pharmacist will work with you on the areas that matter most—such as medication safety, CGM support, healthy eating, problem solving, and optional InBody progress scans for body composition and muscle tracking.
4) Ongoing Support Between Visits
We’ll continue checking in through prescription pickups, scheduled follow-ups, phone calls, CGM support, and progress tracking so you feel supported between doctor appointments.
5) Updates Shared With Your Provider
When appropriate, we’ll communicate progress, concerns, and recommendations back to your provider so your care team stays connected.
