This program is for eligible Medicare patients with high blood pressure.
You may receive a smart blood pressure monitor to use at home. The monitor can save and send your readings automatically, so your blood pressure can be followed over time.
This can help you and your care team better understand your blood pressure between doctor visits.

2. Pharmacy RTM Program
Medication Adherence Support
This program is for eligible Medicare patients who may benefit from extra support taking medications regularly.
Medsense may use small sensors, medication containers, a pill organizer, or a smart box to help track when medications are taken. Patients can also receive reminders by text message or phone call.
This can help reduce stress around remembering medications and may help the pharmacy team better support you.

What Happens Next?
If you are interested, Medsense may schedule a short call with a physician.
This call usually takes about 15–20 minutes. It helps confirm that you qualify for the program.
After that, Medsense can help send the equipment to you and walk you through setup.
