1. PGE1 (Alprostadil)- the single-agent option
Alprostadil, also known as Prostaglandin E1 (PGE1), is a naturally occurring substance that helps widen blood vessels. When injected directly into the penis, it works by relaxing the smooth muscle and increasing blood flow, which leads to an erection.
This treatment is often recommended for men who don’t respond well to oral medications for erectile dysfunction (ED) or who can’t tolerate their side effects. 2. TRIMIX- THE MOST COMMON MEDICATION
Trimix is a compounded medication that combines three drugs: Alprostadil (PGE1), Papaverine, and Phentolamine. Together, these medications work by relaxing various parts of the penile tissue and blood vessels, significantly improving blood flow to produce an erection. Trimix is often prescribed for men who require stronger or more consistent results than alprostadil alone can provide. It’s widely used in erectile dysfunction (ED) clinics and compounding pharmacies due to its high effectiveness. However, since it must be specially prepared by a compounding pharmacy, access may be more limited, and there’s a higher risk of side effects. Most notably priapism, a prolonged and potentially harmful erection.
3. QUADMIX – THE STRONGEST BLEND
- What it is: A more potent version of Trimix, Quadmix adds a fourth ingredient, typically Atropine or another enhancer.
- How it works: The fourth agent boosts blood flow even more or extends the duration of the effect.
- Who it’s for: Patients with severe ED who didn’t respond well to Trimix.
- Pros: Strongest response for men with challenging cases.
- Cons: Higher risk of priapism and side effects; careful dosing is essential.
4. REVERSE AGENT- USED WHEN ERECTION LASTS TOO LONG
- What it is: Usually Phenylephrine, an injectable alpha-agonist that constricts blood vessels.
- Why it’s used: To reverse an erection that lasts too long (priapism), which can be dangerous if untreated.
- Who it’s for: Men using injection therapies who may be at risk of prolonged erections.
- Key Tip: If your erection lasts more than 4 hours, this is a medical emergency—do not wait. Call your provider or go to the ER.
