Grant-funded medication monitoring and quality improvement support through Skippack Pharmacy
Skippack Pharmacy is proud to be 1 of only 20 pharmacies nationwide selected for a grant-funded diabetes quality improvement initiative in partnership with the National Community Pharmacy Association and Community Pharmacy Enhanced Services Network.
This program is focused on helping select patients using GLP-1s, insulin, and oral diabetes medications stay safer and get better results between office visits.
What We Monitor
- A1C and blood sugar goals
- Side effects and tolerability
- Refill adherence
- Dose increase or decrease opportunities
- Medication safety concerns
Because patients may visit the pharmacy up to 35 times each year, our team can identify concerns earlier and work with providers to improve medication safety and effectiveness.
